Seeing My Primary Care Physician:
Every effort is made to schedule your child with his or her primary care physician, however there may be times when we are unable to accommodate this request. This could occur after hours, on weekends or on Holidays.
Bilingual Support Services:
We have a number of staff who speak Spanish. If you need help with other languages or sign language, please let us know and we will make every effort to ensure your needs are met. The Salem Pediatric clinic has access to resources to support hearing impaired parents or patients and provide translation for other languages with our Interpreter services.
Prescriptions:
When you have a prescription that needs to be refilled, call your pharmacy first as this will expedite the refill process. If you need to speak to someone regarding your prescription call our office number (503) 362-2481 and choose option 2. Follow the prompts to locate your physician and then choose option 2 to leave a voicemail on their prescription line. Please include all the information requested on this voicemail and remember we require 48 hours to refill prescriptions.
Referrals:
When you are referred to another physician for care, our referral coordinator will work to obtain any necessary authorizations from your insurance company. The paperwork and process associated with a referral can sometimes take a week to 10 days to complete, so check with your insurance company immediately to find out if this is a requirement.
Leaving the Practice:
It is our policy to provide care up to the age of 21 years or until the patient graduates from college. Exceptions to this policy need physician approval. We recommend patients leaving our practice sign a medical record release to the new physician as soon as possible.
Medical Record ReleaseTermination:
Our office reserves the right to terminate care. We make every effort to resolve any issues with our families before the clinic makes this decision.
